PACO Rolls Out Across Cheshire & Merseyside to Transform Primary Care

GP practices across Cheshire & Merseyside are set to benefit from the deployment of PACO (Patient and Care Optimiser) as part of an NHS Primary Care Pilot led by the Cheshire and Merseyside Integrated Care Board (ICB). Designed to modernise primary care at scale, PACO aims to enhance patient access, streamline administrative workflows, and support neighbourhood-based care.
The rollout of PACO across Cheshire and Merseyside is a key step in delivering the NHS Elective Care Reform Plan, which aims to reduce waiting lists, improve patient access, and enhance coordination between healthcare providers. By enabling patient self-management, supporting multidisciplinary teams (MDTs), and leveraging data-driven insights, PACO is driving the NHS’s shift toward integrated neighbourhood care. This approach prioritises continuity, prevention, and personalised care, ensuring that patients receive the right support at the right time. By connecting primary care, community services, mental health, and social care, PACO strengthens collaboration across healthcare teams, helping to address population health challenges and create a more efficient, patient-centred system.
